Please complete the online registration ASAP regardless of whether or not you have an appointment scheduled for the medical/physical. Those forms may be turned in anytime over the summer.

In order to complete the online/in-person registration you will need the following:
1. Birth Certificate
2.Proof of Residency
3. Parent/Guardian Driver's License
4. Immunizations
Once you have submitted your registration online you will receive a link to schedule an in-person appointment.
